Why are you contacting me?
We're getting in touch because a child you teach or know at school has taken part in the National Study of Health and Wellbeing: Children and Young People.
The child and their parent/carer has given us your name and your school's name as someone they feel knows the child the most and they are happy for you to fill in a questionnaire about the child. The information you give will allow us to build a fuller picture of children's health and wellbeing needs.
What will happen to any information I give?
Children and their parents/carers will not be able to find out the answers which you give in the questionnaire. They have been informed that this is the case.
Your answers will be kept completely confidential, in accordance with the Data Protection Act 1998, and will be used for research purposes only.
The online questionnaire is hosted on a secure website (HTTPS). This is the same type of website that you would usually use when shopping online.
As part of our commitment to the security of your information, we have regular internal and external audits of its information security, and are accredited to the International Standard for Information Security, ISO 27001:2013.
How can I complete the survey?
You will have received an email from us, as well as a letter with a paper questionnaire enclosed.
You can complete the paper version and return it in the pre-paid envelope. Before you return it please detach the front page of the questionnaire, thank you.
Alternatively you can complete the questionnaire online using the Access Code which you will have received from us in the letter that we sent to you.
Do I have to take part?
We very much hope you will take part but participation is voluntary. If there are any questions you do not wish to answer, you do not have to. If you do not wish to be contacted about the study, please contact us by phone or email.
Do I have to complete the online questionnaire in one go?
It should only take 15 minutes to complete, but if you need to exit the questionnaire before completing it you can return to where you left off. To come back to the questionnaire all you need to do is click on the link in your email.

To protect the confidentiality of your answers, the questionnaire is closed down if it is left for a period of time. Your answers will have been saved so you can log back in by clicking on the link we sent you in the email.
I have completed my questionnaire, but you sent me an email to remind me to do this.
Thank you for taking part! It might be that you completed the questionnaire after we had checked our records. We apologise, but we will have safely received you answers.
